I would think it would have something to do with her 8th. Like transit jupiter going through her 8th or something (major transit) activating her moon (8th house ruler.)

Edit: her progressed moon (ruler of her 8th) was transiting her progressed 12th house and conjunct natal Pluto.

Trying to take this once chart at a time, but I think we have an answer already with her Natal Sun ONLY aspected by a tMars square and all the hard aspects to her 8th ruler, Moon.

That would make sense as the sun rules the heart and you said she died of a heart condition.
